Drug class: Selective estrogen metabolism modulator

Mechanism of Action:

DIM is a metabolite formed from indole-3-carbinol (I3C), a compound naturally present in cruciferous vegetables (e.g., broccoli, cabbage, Brussels sprouts).
DIM acts mainly by modulating estrogen metabolism rather than directly increasing or decreasing estrogen synthesis. It influences cytochrome P450–dependent pathways, particularly CYP1A1 and CYP1A2, promoting the hydroxylation of estrogens toward relatively less estrogenically active metabolites such as 2-hydroxyestrone (2-OHE1) and reducing the relative proportion of more potent estrogen metabolites such as 16α-hydroxyestrone (16α-OHE1)

DIM may also interact with estrogen receptors (ERα and ERβ), producing tissue-dependent estrogen-modulating effects. Additional proposed mechanisms include regulation of antioxidant and inflammatory pathways and modulation of enzymes involved in steroid hormone metabolism

Effect on fertility biomarkers

The effect of DIM on female fertility has not been established in controlled clinical trials. By modifying estrogen metabolism, DIM may theoretically influence reproductive function in women with conditions associated with estrogen excess, such as estrogen-dominant hormonal patterns. However, excessive reduction of estrogen signaling may potentially have unfavorable effects, including reduced endometrial proliferation, impaired cervical mucus quality, or altered ovulatory conditions in susceptible women. Because adequate estrogen activity is required for follicular development, ovulation, endometrial receptivity, and production of fertile cervical mucus, DIM supplementation during active attempts to conceive should be approached cautiously. Current evidence does not demonstrate that DIM improves pregnancy rates or fertility outcomes. Its possible effects on fertility are likely dependent on baseline estrogen status, dose, and duration of use. A possible mechanism affecting cervical mucus is related to reduced estrogenic stimulation of cervical glands. Since estrogen promotes production of fertile cervical mucus (increased volume, hydration, and sperm permeability), DIM-induced reduction of estrogen activity could theoretically decrease mucus quantity or alter its characteristics. However, direct clinical studies confirming an effect of DIM on cervical mucus are lacking.
